We create a tailor made care plan for you following an initial consultation in your own home detailing the support you want, routines, likes & dislikes, medication support etc. We will also do a complete risk assessment, making sure the environment is safe.
Your Care Support Manager will introduce you to your Care Professional on their first visit & run through your care/support plan with them.
Your Care Support Manager will be in regular contact to discuss the support you or your loved one is receiving. This regular contact enables us to make sure we are doing our best to fulfill any changes in needs & make sure the care we provide is safe and effective.
