All our services are available to adults over 18yrs. Our support can be ongoing/long term or short term, after a hospital stay or as respite, enabling a family career to visit friends or family.
We create a personalised care plan tailored to your needs after an initial consultation in your home. This plan will cover your preferred routines, likes and dislikes, medication support, and more, along with a full risk assessment to ensure your environment is safe. We ensure we match you with a Care professional who has experience in your specific needs.
Your Care Support Manager will introduce you to your Care Professional during their first visit and go over the care plan together.
We maintain regular contact with you to discuss the support being provided, ensuring we adjust to any changes in needs. This helps us ensure that the care we deliver remains both safe and effective.
