What Is a Care Plan?
At Home Instead, every client starts with a personalised Care Plan, tailored to their individual needs and preferences. As circumstances evolve—whether due to a service review every six months, a fall, or a new diagnosis—we update the plan to ensure it remains relevant and effective.
A Care Plan serves as a guide, helping our carers integrate seamlessly into your daily life while providing the support needed to maintain your safety and independence. This support can range from a one-hour weekly visit to assist with light housekeeping, errands, or appointments to around-the-clock live-in care for help with tasks like washing, dressing, meal preparation, and medication management. With a carefully curated Care Plan, we create a structured approach that adapts to your unique situation, ensuring consistent and compassionate care.
