Community Mental Health Team - Our West Lancashire Community Stars of the Month (January 2025)

We are extremely proud to present the Community Mental Health Team (Older Adults) as our West Lancashire Community Stars of the Month for January 2025!

The Older Adult Community Mental Health Team (OA CMHT) provides exceptional support to individuals over the age of 65 with severe and enduring mental health needs, as well as those under 65 with early-onset Dementia. Their unwavering dedication to improving the quality of life for older adults in our community is truly inspiring.

Their work goes beyond mental health care, supporting individuals to remain in their own homes for as long as possible, empowering them to live independently. By working closely with health and social care professionals, they ensure that personalised care plans are in place, helping clients achieve their desired outcomes.

At Home Instead West Lancashire and Chorley, we are excited to continue our partnership with the OA CMHT and others in the community, as we work together to build a brighter, more supportive future for all.
