1. A client reported feeling unwell. This client felt dizzy, sick, lost appetite for food and wasn’t usual self.
2. We promptly contacted the client’s GP, resulting in a paramedic visit, though initial checks found nothing alarming.
3. Our Care Professional’s continued to observe and report ongoing symptoms.
4. Our management team, reviewing these reports, noticed similarities from previous experience with another client who had been diagnosed with low sodium.
5. We suggested this possibility to the GP, who we pressed to arrange blood tests for the client.
6. The tests confirmed low sodium levels, allowing for early intervention and home-based treatment.

This proactive approach prevented a potential hospital admission, as severe cases of low sodium often require intravenous treatment in a hospital setting and a short-term hospital stay.
