Ways to Wellness is a free service in Newcastle providing non-medical support to help you take steps towards living well.

What is Ways to Wellness?

Ways to Wellness is a service for people in Newcastle upon Tyne who are registered with participating GP practices, and whose daily lives are affected by certain long-term condition. GP’s and their teams can refer patients to this service.

The service offers non-medical support to complement your GP’s work. They can help you feel more confident to manage your health and wellbeing by focusing on what matters to you and enabling you to make positive changes.

Who is it for?

The Ways to Wellness service is for people with certain long-term health conditions, aged 18-74, who attend participating GP practices in Newcastle upon Tyne. Please ask your GP or go their Website to find out if your GP is included. The eligible long-term health conditions are:

  • Chronic Breathing Difficulties (COPD or Asthma
  • Diabetes (Type 1 or Type 2)
  • Heart Disease
  • Epilepsy
  • Thinning of the Bones (Osteoporosis)
  • Hypertension
  • Pre-Diabetes (Non-Diabetic Hyperglycaemia)
  • Osteoarthritis of the Hip/Knee

How Does it Work?

A Ways to Wellness link worker will help you identify areas in your life that could be impacting your health and wellbeing and enable you to take steps towards living well. They will work with you to produce an agreed action plan, which may include a range of activities, such as:

  • Getting involved in local groups and activities
  • Developing positive relationships
  • Accessing specialist services and support
  • Healthy eating and diet
  • Getting more active
  • Getting support around benefits and housing
