Welcome Home Mrs D: Home From Hospital

At Home Instead Newport, Chepstow & Cwmbran, we believe that home is where the heart is—and for our client, Mrs. D, returning home as soon as possible was essential for her well-being.

On Sunday, Mrs. D was admitted to the hospital due to a sudden illness. While she received medical care, her overall well-being began to decline, as often happens when older adults spend extended time in a hospital setting. Thankfully, she no longer needed hospital treatment, but a fast and well-coordinated discharge was crucial to ensure she could safely return home with the right support.

By Tuesday, the hospital discharge team reached out to us, explaining the urgency of the situation. Within just 30 minutes of that call, our Registered Care Manager, Jo, was at Mrs. D’s bedside, assessing her care needs alongside her family. It was clear that her previous care plan of one visit per day would no longer be enough—so our team worked at record speed to increase her support to four visits daily.

Behind the scenes, our incredible office team came together seamlessly: Bonnie, our Scheduler, expertly balanced staff availability to ensure we had the right people in place; Cindy, our Care Coordinator, swiftly arranged support for the new care schedule and confirmed everything with our Care Professionals; Christina, another of our brilliant Care Coordinators, logged the updated care plan activities ready for the family to use our internal care app, and Charlotte, our Trainee Care Coordinator, ensured every detail was recorded in our system ready for auditing.

By Wednesday morning, just a day after the initial call, our dedicated Care Professionals, Charlotte and Esther, were at Mrs. D’s home, providing the personal care and support she needed to feel safe, comfortable, and cared for.

Her daughter shared her heartfelt gratitude:
“We were so worried about Mum being in hospital for too long, but the Home Instead team acted so quickly to get her back home with the right care in place. The difference in her well-being in just a day was incredible. We can’t thank them enough for their kindness and efficiency.”
