Our team met with him and the family that afternoon and swiftly arranged for one of our experienced Care Professionals to visit him.

On Tuesday, our Care Manager introduced our chosen Care Professional but came back to the office with concerns about the client’s health which had deteriorated significantly overnight. He appeared more frail and disoriented than initially assessed. As a team, we took these observations seriously and immediately spoke with him and his family. It became clear that he needed more comprehensive support to ensure his safety and well-being. By the end of the day, we had arranged for waking night support and additional afternoon respite visits, along with 2-1 personal care starting immediately.

Wednesday: Seamless Transition to Intensive Care

Wednesday was all about mobilising resources. Thanks to our well-trained and versatile team, we seamlessly increased the client’s care. Our dedicated Care Professionals took on the waking night shifts, while two other Care Professionals provided 2-1 personal care during the day. Additionally, we arranged for further afternoon respite visits to ensure comprehensive support throughout the day.

Our administrative team ensured all necessary documentation was updated, and our care coordinators maintained constant communication with the family, keeping them informed and reassured. The family expressed their immense gratitude for the swift and efficient response.
