Client Background: Our client began receiving care from Home Instead South Cheshire in 2022, recognising the need for support to maintain her independence at home. Although she has family who visit regularly and provide additional help, they live far away, so professional care was essential. Initially, she required three visits per day – morning, lunchtime and evening – to assist with getting up, preparing meals, and managing her nighttime routine. Additionally, she enjoyed short walks and gardening with her Care Professionals, and it is clear that building strong relationships with them was incredibly important to her.
As her needs evolved over time, the level of support required also increased. Home Instead responded by adapting the care plan to ensure she had the support necessary to continue living comfortably at home.
Transition to 24 – Hour Care: After a decline in health and subsequent hospital stay, our client’s needs became more complex. It was decided that she would benefit from 24-hour care, which would allow her to remain at home safely. We provided Care Professionals in shifts throughout the day, along with a Care Professional staying overnight. This trial was successful, and our client, who was adamant about not wanting to go into residential care, expressed her desire to continue living at home with full-time support.
Switching to Live-In Care: To provide the most cost-effective and sustainable solution, we offered our client the option to transition to Live-In Care. After careful consideration and matching, we introduced two Care Professionals – Abbie-Leigh and Cassie – who now provide care on a rotating two-week schedule, ensuring consistency and continuity.
Building Strong Relationships: Abbie-Leigh was the first to move in with our client. Over the course of their initial days together, they began to form a wonderful relationship, bonding over shared interests such as wildlife documentaries and literature. Abbie-Leigh is also trying to encourage our client to write her life story, giving her a creative outlet and helping her reminisce on her cherished memories. Their bond grew stronger with each passing day.
After two weeks, Cassie was introduced into the care team. Cassie quickly connected with our client, bonding over their mutual love of documentaries. Cassie also took extra steps to enhance our client’s comfort, such as buying a cup that would keep drinks warm for longer and was easier for drink from. Cassie even bought a matching cup for herself so that they could enjoy their time together with a sense of shared experience. The thoughtful details like this have made a significant impact on the client’s comfort and happiness.
The relationship between the two Care Professionals also have flourished. Abbie-Leigh and Cassie have developed a warm rapport, sharing stories and tips during their handovers. Their bond, along with their mutual dedication to providing the best care for our client, is evident in every aspect of their work.
Positive Feedback from Family: Our client’s niece shared her gratitude, saying that the Care Professionals were carefully matched to her relative, and that it was evident that the placement was working exceptionally well. She highlighted how the care team has not only provided excellent care but also brought peace of mind to the family, knowing that their loved one is safe, comfortable, and well looked after.
The niece added, “I have met all of the Care Professionals who have looked after my relative over the years, and all of them have been lovely. They genuinely care about their clients.”
