One of the benefits of having a fully managed care service is the way that we can work with other health professionals such as district nurses, pharmacies and physios. Often our clients are not able to visit their pharmacy and family members either live some distance away or find it difficult to make extra trips because of family and work commitments. At Home Instead, we understand how important it is to make sure prescriptions are well managed and our care team are well-practiced at stepping to help manage this vital relationship with your local pharmacy. This can be particularly important when in transition between hospital and home or if family members are on holiday. Communication can be crucial at these times, and can be particularly difficult for people with Alzheimer’s and other forms of dementia, so clients find peace of mind in the fact it is all taken care of and by asking the right questions, we can help clients to understand what their medication is for and help them to manage it better.
Some clients do like to speak to a pharmacist in person so our Care Professionals regularly take clients to their pharmacy to facilitate the discussion, to arrange tests such as blood pressure tests or to receive vaccinations such as the winter flu jab. Case Study
A client was given whole tablets with a prescribed dose of half a tablet to be given daily. Her regular pharmacy refused to cut them as it takes up too much time, but as caregivers, we cannot cut medication, so we then had to look around to find a pharmacy that would do this. It took a while to find one that would do this for her, but it was well worth the effort and after finally finding a pharmacy that would pre-cut them our client moved to that pharmacy for all of her medication, giving peace of mind to our client and saving a lot of time for the family.
