A real life example from Home Instead Yeovil, Sherborne and Bridport showcasing how we achieve continuity in Home care.
At Home Instead Yeovil, we know that continuity in home care is crucial in building strong, trusting relationships between our clients and Care Professionals. Matching clients with the right Care Pro and ensuring consistency in their visits isn’t just about comfort – it’s about creating meaningful bonds and achieving positive outcomes that truly make a difference in our clients’ lives.
For clients with unique needs, such as Jenny, who has hearing difficulties and is partially blind, continuity is especially important. Her husband, who is also in poor health, receives our home help with domestic duties and cooking. Given their circumstances, maintaining familiarity and trust in the support they receive is essential.
Since May, Maria has become Jenny’s regular Care Professional, thanks to the effort of our dedicated scheduler, Skye, who ensured Maria would visit Jenny consistently. The continuity of having a familiar face during each visit allowed Maria and Jenny to form a trusting relationship – something incredibly valuable when navigating daily routines and overcoming challenges.
The significance of this consistent relationship became clear when Maria helped Jenny reach a major personal milestone. Jenny had not been able to wash her hair for a long time due to her health challenges. However, after building rapport and trust, Maria was able to assist her in having her hair washed – a seemingly small but powerful moment that restored comfort and dignity to Jenny’s life. This was also a planned outcome for Jenny, and seeing it come to life was truly heartwarming.
Now, thanks to the bond that has formed, more visit time is allocated to regularly support Jenny with her hair care needs. This outcome is a testament to the value of continuity, proper matching, and the commitment of our team to deliver personalised, compassionate home care.
An additional benefit of continuity in care is the ability to notice when a client is not quite themselves. In such situations, taking early steps like arranging for a urine sample to be taken to the GP can lead to timely diagnosis and treatment, such as antibiotics, which could potentially prevent a hospital admission. This proactive approach not only ensures the client’s health is prioritised but also reduces the stress associated with hospital visits.
It’s a bond like a heart – the client and Care Professional on one side, and the scheduling and office team support on the other, making a full circle. Together, these connections ensure the best possible home care and support for our clients, driven by a team that truly cares.
At Home Instead Yeovil, we believe that by matching clients with the right Care Professionals and scheduling regular visits, we can nurture these relationships and provide exceptional home care that makes a real difference. Continuity in care ensures clients feel more comfortable and creates the environment needed for meaningful and impactful outcomes.
If you or a loved one needs care at home and want to experience the difference continuity in care can make, feel free to reach out to us. At Home Instead, we’re here to help, with a team dedicated to making life at home safe, comfortable, and filled with care.
