The care consultation

At Home Instead, we understand that it may be a difficult decision to start receiving care and support at home. We also know how important it is to make sure that the care is provided in the way that is right for you.

What is a Care Consultation?

A Care Consultation is a thorough assessment of care and support needs.

An experienced member of our team, usually the Care Manager, will visit the person looking for care and support in their home to complete the consultation with you, your family and anyone else who is important such as advocates or friends.

During the Care Consultation we will ask you to share what is important to you, find out about your life history, health needs, daily routine and lifestyle. We can then work with you to create a care plan that sets out how we will support you to achieve your goals and remain independent in your own home.

Gathering this information enables us to select Care Professionals to support you who closely match your interests, hobbies and personality so they can become friendly faces that you will get to know and trust.

We will make sure that we provide you with and explain key information about Home Instead including our Statement of Purpose. We will also give you important information about how we enable you to keep safe.

We will talk to you about how we use digital systems to ensure that care plans are updated in real time and medication is managed safely.

After the Care Consultation

We will create a bespoke Care Plan that sets out how we will deliver care and support for your loved one, focussing on the outcomes you want to achieve from our care.

Based on the information they have shared about their interests, hobbies and personality, we will select carefully matched Care Professionals to provide their support so they can become friendly faces that they and you will get to know well and trust.

During the Care Consultation we will tell you if we are able to meet your needs. We will agree the amount and type of visits to be provided and agree the start date with you.

As part of the Consultation, we will check that you understand and consent to the service being provided and a Service Agreement (contract) will need to be signed, either at the Care Consultation or afterwards.

We will also need to create your Care Plan and identify the right Care Professionals before your service can commence.
